It appears he has an approximately 10 year old daughter, Ju-ae. There is speculation about the possibility of other children but I can see why they'd want an adult to take over.

Adding this really interesting documentary link from the BBC, who partnered with specialist news sources to talk to defectors and those inside the country. At least until NK recently started partnering with Russia to make weapons (and get some aid from Russia), they were experiencing a famine nearly as bad as in the 1990s.

 

There have also been signs of mild dissent. From reading NKnews (insider and defector sources), I have heard that a law enforcement officer leaked documents showing a problem; in the past year or two, desperate citizens have either been ignoring or physically fighting street-level police, who often do not have working guns, when those officers tell them they can't forage for food or engage in survival-level trading. Also, this was poignant and mentioned by a source in a provincial capital: an elderly man who had always been loyal was slowly starving. So he went to the local government office, sat down, and started yelling, "I have no food, Kim said he will take care of us, give me food."

 

The source was unclear what happened to the old man.
