Since the swastika is made of syringes, I'm assuming that it's a statement comparing the push for vaccination to the Nazis, not a statement in support of the Nazis.  

Still problematic.  Slightly different problem.  More of a person who doesn't see the difference between carrying a vaccination card and wearing a Star of David.
